Policies

As our patient, we want to familiarize you with some of our policies and practices relating to patient care. We hope you will find the information helpful in answering some of the questions you might have.


CANCELLATION/NO-SHOW POLICY

For all scheduled medical visits, if you miss an appointment without a 24-hour notice, cancel the same day as your appointment, or fail to arrive to your appointment at the scheduled time, a $75.00 cancellation/no-show fee will be assessed to your account.  

For all cosmetic or surgical visits, f you miss an appointment without a 24-hour notice, cancel the same day as your appointment, or fail to arrive to your appointment at the scheduled time, a $100.00 cancellation/no-show fee will be assessed to your account. 

Payment of this fee must be made before further appointments are allowed to be scheduled. We will make every effort to remind you of your appointment via email and/or phone, but please help us by always making sure your phone numbers and email address are updated with us during visits. 

LATE ARRIVAL POLICY

We strive to see patients at their scheduled appointment time. Therefore, if you are more than 10 minutes late, you may be asked to reschedule your appointment so that other patients are not inconvenienced.

PAYMENTS

Payment is due at the time of your appointment for services performed, including copay, coinsurance, patient balances, and unmet deductibles depending on your plan's benefits. We accept most forms of payment including cash, check, debit, and credit (Visa, Master Card, Discover, and American Express). We also have a credit card on file (CCOF) policy to allow for smoother transactions. The credit is stored safely and held on a secure site with our third party merchant service processor, Square. Your credit card information will not be held in our office electronic health record so no financial information will be attached to your file. 

PRESCRIPTIONS

If you find the cost to fill your prescription(s) is too high, please contact our office so that we may provide you with a less expensive alternative.  we will recommend a specialty pharmacy that assists you in completing the prior authorization.  Please be advised that our office does not complete prior authorizations for medications, but we are happy to recommend a specialty pharmacy that can assist you in completing the prior authorization.  Prior authorization process takes up to 7 business days to complete and sometimes longer depending on the specific medication.  Should you have any questions about your prescriptions please contact our office for assistance.

PRESCRIPTION REFILLS

If you would like a prescription refill, please have the pharmacy send us a request electronically or ask the pharmacy to fax us a request at 513-715-0003 or and allow up to 72 hours for it to be processed.

LAB TESTS

It is sometimes necessary to order lab tests in order to obtain additional information about your condition and/or to monitor the effects of treatment. If lab work is required prior to a return visit to our office, we do ask that you complete it at least 48 hours in advance to allow ample time for the lab to process and send results to our office. It is your responsibility to ensure that the results of any lab testing are received by our office prior to your scheduled appointment. Results may be faxed to us or you may bring them with you. 

MINORS

The parent(s) or guardian(s) of minor patients (under the age of 18) MUST accompany the child for all medical visits and sign an informed consent to treat the child.  It is the responsibility of the parent or guardian to provide current insurance information and payment in full for services provided.
