What to Expect
Your first visit to our Cincinnati office is designed to get you better acquainted with our office as well as introduce you to our caring staff. We encourage questions and do our best to always deliver quality care.
Here are a few things that you can do to help prepare for your visit:
- Please bring your photo ID, preferred form of payment, and insurance information/card to all of your visits, (see Insurance and Billing section for more information)
- Please remove all makeup and nail polish before your visit
- Please bring an up-to-date list of all your current medications, supplements, and allergies.
- We prefer that a parent accompany adolescents (younger than 18 years of age) for their visits.
Patient Forms
In order for our staff to prepare for your office visit, please fill out the patient forms that are sent from our office via SMS/text message using a secure link. This will help streamline the check-in process and ensure our files are up-to-date. You will also receive an email from [email protected] to create a login for our Sadio patient portal after your requested appointment has been confirmed. The Sadio patient portal will contain important after-visit counseling notes for your reference and pathology report(s) as applicable.
