If, for any reason you cannot keep a scheduled appointment, or will be delayed, please call us as soon as possible. Please arrive 15 minutes prior to your appointment and bring all current medications or supplements, up-to-date medical insurance information, driver's license and a form of payment to keep on file (credit card, check, HSA or flex-spending card).

We are happy to file insurance for your reimbursement as long as you are free to choose your own dermatologist.
